Pricing built for the long tail

Start with a full vendor management system (VMS) at $0 to your facility, then add compliance, recruiting, and onboarding modules only when you need them. Every fee — including the agency-funded core and the capped Network placement fee — is disclosed openly.

Five tiers, one platform

Enterprise VMS and managed service provider (MSP) contracts are priced for large integrated networks — six figures a year, long procurement cycles. Health Shift Hero packages the same operational control for independently owned skilled nursing facilities (SNFs), assisted living, dialysis, surgery centers, and private-duty operators, at long-tail price points.

Tier Buyer Price Positioning Key inclusions
VMS Core Facility + agencies $0 facility fee
+ 2% of agency gross billings
(supplier-funded)
A free facility VMS for long-tail facilities, funded by participating agencies. Agency portal, facility portal, worker profile portal, credential file management, invoice/payment workflow, agency compliance packets, monthly Office of Inspector General (OIG)/sanctions re-screening, credential-expiry gating (a worker with a lapsed required credential cannot be confirmed onto a shift), facility-specific requirement rules, full audit trail.
Compliance Audit Facility $300/mo For facilities that want to manage their own employee compliance, not just agency staff. Employee credential records with full history, expiring-credential tracking and reports, monthly OIG/sanctions re-screening, background-check ordering (pass-through), audit-ready document vault, direct-hire onboarding records, basic custom forms.
Network Facility $900/mo
+ 2% placement fee
Turns the facility into its own local talent network. Everything in Audit, plus facility job-posting page, QR-code application links, shift bidding and availability capture, internal float pool, candidate pipeline, applicant workflow, and an agency-vs-direct comparison dashboard.
AI Intake & Onboarding Facility or agency +$300/mo add-on Reduces admin work on applications, resumes, and onboarding packets. Resume parsing, skills extraction, credential matching, field mapping, missing-field flags, and onboarding-packet prep — routed to the right person for proper human completion and signature. Document-checklist automation and a monthly credit pool for AI-assisted screening.
Premier Workforce Hub Facility $1,500/mo Full facility workforce command center for agency labor, direct hires, compliance, and recruiting. VMS Core + Audit + Network + AI Intake, plus advanced reporting, vendor scorecards, rate analytics, compliance exception dashboards, rule templates by department, onboarding workflow customization, and priority support.

AI Intake & Onboarding is packet prep, not filing. The AI parses resumes, maps fields, flags missing information, and assembles onboarding packets, then routes them to the appropriate person for completion and signature. It does not complete or file I-9s, tax forms, or any government paperwork on your behalf — a human always finishes and signs.

What is live today

The tiers above are the packaging. Not all of it is built yet, and we would rather tell you that here than in a demo.

Live now On the roadmap
The agency gateway and facility portal; credential file management with facility-specific requirement rules; compliance gating that refuses to confirm a worker whose required credential has lapsed; invoice audit against worked-and-approved hours; statements; disputes and credit memos; timecards with QR and geofence punch; the staffing-minimum (HPRD) dashboard; CMS Payroll-Based Journal export; the survey binder export; invited onboarding and the counterparty claim flow; monthly OIG/LEIE and Louisiana adverse-actions re-screening of every credentialed worker, with automatic blocking on a confirmed exclusion and human adjudication of possible name matches before anything blocks. SAM.gov and additional state exclusion lists (the OIG/LEIE and Louisiana lists are live); background-check ordering; AI Intake & Onboarding; vendor scorecards and rate analytics in Premier; applicant-to-hire conversion and placement-fee metering in Network.

No money moves through the platform. Statements, invoice audit, approvals and payout records are workflow and bookkeeping only — settlement stays between the facility and the agency. Prices are the current v2 packaging and are subject to confirmation.

How the Network take-rate works

The Network placement fee is disclosed up front and is deliberately structured to sit well below traditional recruiter and marketplace norms.

Placement type Take-rate Cap
W2 direct hire 2% of gross wages First 460 worked hours only. No further fee on that hire after the cap.
Per-diem / 1099 shift 2% of total shift price Per shift; no cap.

For a W2 direct hire, the 2% placement fee applies to gross wages for the first 460 worked hours only — roughly three 12-hour shifts a week over a quarter — after which no further placement fee is charged on that hire. Per-diem and 1099 placements are charged 2% of the total shift price per shift, with no cap.

Straight answers

Who pays for VMS Core?

Participating staffing agencies fund VMS Core at 2% of their gross billings. The facility pays $0 for the core platform. The 2% agency fee is disclosed openly on every agency's statements — it is a selling point, not fine print.

Can we start free and add modules later?

Yes. VMS Core stands on its own at no facility cost. Compliance Audit, Network, AI Intake & Onboarding, and the Premier Workforce Hub are optional monthly modules you add as your operation grows — no re-platforming required.

Do you take a cut of Audit or AI Intake?

No. Compliance Audit and AI Intake & Onboarding are flat monthly software fees with no take-rate. The only percentage fees on the platform are the 2% agency-funded VMS Core fee and the capped Network placement fee — both disclosed above.
